Care Mobile Clinic Open to ALL Students!

Date: August 7, 2026
Time: 9:00 AM - 3:00 PM (First-come, first-served)
Location: Evelyn Alexander School

Visit your child’s school office to complete and sign the required consent form.

Provide a copy of your child's immunization record.

Note: Parents/guardians must complete the appropriate sections of the physical and consent forms prior to the clinic. The team will see as many students as possible, but no students will be accepted after 3:00 PM.

You can visit the health clinic at North Chicago Community High School between 9:00 AM and 1:00 PM to get this done. The next clinic dates are August 20 and September 9.
